generate-id() coupled with key() is only going to give you the first instance of each distinct element in a group. With that information you can then test each of the elements against this information and group them as such. I wish I had the time to look through your particular code a bit more and figure out your specific error. But I do have the time to copy and paste a quick and simple example that highlights the correct process using apply-templates and a passed parameter containing all of the ungrouped elements that can then be used to test against each distinct group.
