Well that is part of the XSLT reference in the MSXML documentation, also 
online here

If you compare that to <http://www.w3.org/TR/xslt#extension> then you 
see where that description of the attribute comes from. Only a 
description of the attribute does not imply there is mechanism for MSXML 
to implement extension elements.

As said, MSXML has msxsl:script as its own extension element to then 
define your own extension functions, but to my knowledge it does not 
allow you to implement your own extension elements.
