Special character encoding

I have an xml schema with some custom tags inside the xs:documentation
section. I use an XSLT to generate some text and xml out of this. I have set
the output method to "text" and encoding to "utf-8" in the xslt. However, I
want that any special characters should be xml encoded in the output xml and
text that I generate (& should be encoded to &amp; and so on).

I have tried using disable-output-escaping but it does not seem to work.
Right now I am using the html codes like &#38; etc. but this is not user
friendly.

Is there any way out?
