XSLT2 and Omnimark are two languages with pattern-matching features that can do this, provided your plaintext file is sufficiently regular that the patterns of structure can be programmed. Both are in widespread use: XLST2 is available in free implementations; Omnimark is a commercial product. It's also possible to use the standard scripting languages (Perl, Python, Tcl, etc) to do this, often in conjunction with the standard Unix shell text tools (available for Windows from www.cygwin.com). The scripting languages all have extensive XML libraries, but these are mostly for writing applications that have XML *input*.
