It sounds as if you need to write a program to collect the information
from the user and output an XML document containing that data. But there
are lots of ways to speed this up. If all your users use Windows, have a 
look at InfoPath. 

If you prefer the user to use a Web browser as their interface, you can
write a HTML page with the data-collection form and use some Javascript
to perform validation and control and express the result as XML provided
the backend data volume is not too large.

But there must be thousands of these applications already out there. It
may be simpler and cheaper just to go out and buy one.
